- Complete a claim fax cover sheet and fax it to 905-507-2882.
- Send all claims details to sarahd.davies@gfh-underwriting.com by clicking the link here.

Please include the following information:
- Insured Full Legal Name
- Insured Contact Number
- Policy Number
- Brokerage Name
- Brokerage Contact Name
- Brokerage Contact Number
- Description and comments for claim
- Any supporting document
